If your mailing address is the same as your permanent address, choose the option "Same as Permanent Address". The next question is, "Are you an agent acting as an intermediary?" Choose "No" and then skip this part. For the tax identification number (TIN), choose "I have a foreign non-US income tax identification number", if you have one. If you don't have a TIN, choose "I don't have a TIN or foreign non-US identification number". Next, if you choose the option of not having a TIN, 30% of your overall income will be withheld for tax purposes.
